Please provide the following information on a SEPARATE SHEET for each upbound voyage via fax to the traffic control centre number listed below at least 96 hours prior westbound entry of the St. Lawrence Seaway at C.I.P 2 in Montreal:

3) Radio Call Sign 11) Date and Time of Pre-entry Form Submittal
4) Date of Arrival in Montreal 12) Provide name and phone number of Agent in Montreal
5) Owner’s Name 13) Estimated date of arrival at Snell Lock
6) Operator’s Name 14) Type of Cargo (Bulk, General, etc.)
7) Last 4 Ports and Dates of Departure 15) Has USCG self-examination been completed? (Maximum 7 days prior to entering Seaway at C.I.P. 2)
List all outstanding items.
Note: The original completed and signed copy is to be retained on board.
