Pre Entry Information Required from foreign Flag Vessels

Please provide the following information on a SEPARATE SHEET for each upbound voyage via fax to the traffic control centre number listed below at least 24 hours prior initial Seaway inspection or 24 hours before westbound entry of the St. Lawrence Seaway at C.I.P 2 in Montreal:

3) Radio Call Sign 11) Date and location of last U.S. Port State Control Exam
4) ETA Montreal 12) Date of expiry of U.S. Certificate of Financial Responsibility
5) Owner’s Name 13) Shipboard Oil Pollution Emergency Plan approved and onboard?
6) Operator’s Name 14)Has USCG self-examination been completed?
(Maximum 7 days prior to entering Seaway at C.I.P. 2)

Note: The original completed and signed copy is to be retained on board.
